As a firm, we DO NOT blame anyone as 99.9999% of the time, issues are due to the process or systems we use. We EXPECT so called ‘mistakes’ and that is OK as no-one is perfect.
We are not mind readers and only by having visibility of the issues, can we (as a team) figure out how to fix stuff.
